New Client Agreement

We love grooming pets and we want to keep the process as straightforward as possible. That being said it is always best for everyone to be on the same page in regards to policy's and procedures we follow here at Benchmark with all of our new friends. We have so rarely had any hiccups but we find it is best to be upfront. We recommend you review the entire agreement before your appointment, feel free to ask questions as at your first appointment it is required you sign the New Client Agreement! Thank you for your understanding and cooperation. 


Rabies Vaccination

All pets are recommended to be current on all vaccinations. State of New York legally requires us to see current proof of the rabies vaccination in order to receive grooming services. Paper, digital or verbal confirmation from a veterinarian is required before any services and proof must be kept current as they expire. (NO TAGS)

Health or Medical Problems

Grooming can expose a hidden medical problem, aggravate a current one, or even expose pet to illness. This can occur during or after grooming. The pet owner acknowledges that they have been informed us of any current or past issues. Any and all medical expenses for veterinary care will be covered by the pet's owner upon signing this agreement. The pet owner releases Benchmark Grooming from any liability and any cost associated with veterinary care of the pet. The pet owner will be financially responsible for any and all medical expenses that may be incurred.

Accidents and Injury

Although accidents are very rare, there is a risk when dealing with animals. Grooming equipment is sharp, and although Benchmark Grooming and its groomers use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when an animal is wiggling, fighting the groomer or moving around.

Your pet’s safety and comfort is our number one priority. In the event an accident does occur, you will be notified of the accident. If Benchmark Grooming feels any medical problem or injury is serious, and the owner is not on-site or immediately available, Benchmark Grooming will seek immediate veterinary care for your pet with the closest vet clinic. PET OWNER HEREBY RELEASES BENCHMARK GROOMING, AGENTS, REPRESENTATIVES OR EMPLOYEES FROM LIABILITY AND WAIVES ANY CLAIM AGAINST

BENCHMARK GROOMING, AGENTS, REPRESENTATIVES OR EMPLOYEES FOR ANY DAMAGES

PHYSICAL, EMOTIONAL, FINANCIAL, OR OTHERWISE RELATED TO PET OR PET OWNER DUE TO ANY CAUSE WHATSOEVER INCLUDING, WITHOUT LIMITATION, NEGLIGENCE ON THE PART OF THE GROOMER.

Veterinarian Authorization

This release form provides Benchmark Grooming and its representatives or its employee full authorization to obtain medical treatment from any of the nearest vet clinics or vet hospitals, in case of any emergency while the pet is present at Benchmark Grooming.

Bordetella Vaccination

Kennel cough is a respiratory condition caused by a variety of viruses and bacteria often spread by direct contact by dogs. At Benchmark Grooming, we strongly recommend that your dog be immunized & kept up to date with their Bordetella (Kennel Cough) vaccine to prevent the spread of kennel cough and/or diminish the severity of infection in your pet. If a pet should become infected, all treatment and costs associated shall be the pet owner’s responsibility.

Flea Treatment

If fleas are found to be present on your pet, an additional charge of $12+ will be added to your service charges for flea bath treatment.

Refusal of Services - Dangerous or Aggressive Animals and Muzzles

Benchmark Grooming and its groomers reserve the right to refuse services in the event that your pet exhibits behavior that its groomers deem to be aggressive or dangerous. If service is refused during the process of rendering grooming services, Pet Owner will only be responsible for grooming services performed up until the refusal to continue grooming service.

Late Pet Pickup, No Show & Cancellation Policy

Benchmark Grooming does not operate as a kennel or dog day care facility.

Failure to retrieve pet within 60 minutes after grooming has been completed will incur LATE FEES, unless previously agreed upon with the groomer in advance.

If you are going to miss your appointment for any reason, please contact us ASAP. We require at least a 24 hour notice for all cancellations or reschedules to allow us time to fill the space. We will hold grooming appointments for 15 minutes past the scheduled drop off time. Late appointment arrival of more than 15 minutes may be declined. No shows and appointments cancelled less than 24 hours in advance may be required to prepay a nonrefundable deposit for the base cost of their pets next scheduled service after the second no show or last minute cancellation.

Photography

This release form authorizes Benchmark Grooming to take photos of pet or pet owner for publication in advertising and any media presence on line for Benchamark Grooming , and waives any rights of pet owner or pet to reimbursement for said photos.
